Standard Scoliosis Treatment in the UK
- Observation
For mild scoliosis, particularly in growing children the first step is usually “wait and see”. This means monitoring the spine through regular check-ups and x-rays to assess whether the curve is worsening. However, scoliosis is a progressive condition, especially during periods of rapid growth like adolescence, so simply observing may not be enough. A more practical approach, such as introducing scoliosis-specific exercises early on, can help halt or slow progression and potentially reduce the need for bracing and surgery.

- Bracing
If the curve progresses to a moderate degree (typically 25-40 degrees), bracing may be recommended in growing children and adolescents with the aim to prevent further progression. Through the NHS the most common bracing type is a Boston brace, a rigid, custom-moulded brace designed to hold the spine in a more aligned position during periods of rapid growth. Patients are usually asked to wear the brace 18-23 hours per day, depending on their individual case and growth stage. Progress is monitored through regular reviews and x-rays.
Whilst bracing can be effective in slowing or halting curve progression, it does not actively correct scoliosis, and success depends heavily on patient compliance and often requires wear for up to 23 hours per day. This prolonged use can lead to muscle weakness, as the brace supports the spine externally without engaging the muscles needed for natural postural control. As a result, when patients are eventually weaned off the brace, it’s not uncommon to see progression of the curve if muscular support hasn’t been developed.

- Surgery
Severe scoliosis (typically curves over 40 degrees) may lead to a recommendation for spinal surgery. This is a major operation where typically metal rods and screws are used to straighten and stabilise the spine. Whilst this method is effective at halting progression and reducing curve sizes, it comes with significant risks, a long recovery period, and a permanent reduction in spinal flexibility. - Pain Management

In the UK, standard medical care for these types of patients often includes painkillers as a first-line response. Patients therefore may be prescribed over the counter painkillers, prescription medications or anti-inflammatory drugs.
